What should you know?
Thyroid nodule risk is not determined by size alone. TI-RADS evaluates composition, echogenicity, shape, margins and echogenic foci.
Ultrasound shows superficial structures such as the thyroid, lymph nodes and soft tissues in high detail. It defines lesion size, composition and relation to surrounding tissues.
Common symptoms and signs
- Solid, cystic or mixed composition
- Echogenicity relative to thyroid tissue
- Taller-than-wide shape and margins
- Calcifications and suspicious lymph nodes
Diagnosis and the role of ultrasound
The radiologist evaluates shape, margins, contents, calcification and blood flow. TI-RADS features are used for thyroid nodules; lymph nodes are assessed by shape, cortex and fatty hilum.
- Size, shape and exact location
- Solid, cystic or mixed composition
- Margins and relation to adjacent tissue
- Doppler flow and risk features
Ultrasound findings must be interpreted together with symptoms, examination and, when needed, laboratory or other imaging results. An image alone should not be used for self-diagnosis or self-treatment.
Treatment and next steps
TI-RADS category and size determine follow-up or biopsy thresholds while clinical risk is also considered. Depending on findings, observation, laboratory testing, specialist review or needle biopsy may be advised. Size alone does not determine management.
Management is individual and depends on age, symptoms, imaging findings, other health conditions and pregnancy plans. Medication or a procedure should be selected with the appropriate clinician.

- Difficulty breathing or swallowing
- A rapidly enlarging, painful or hard mass
- High fever, marked skin redness or severe swelling
